Players under the age of 18 must submit a waiver, signed by their parent or legal guardian, to Twin Mask Logistics Staff before participating. Contact CmsTwinMask@gmail.com to request this form.

• If you are age 16-17: The waiver is enough, and you do not need to be accompanied by a parent or guardian at Twin Mask.
  
  
• If you are age 14-15: In addition to submitting the waiver, you MUST have a parent or guardian on site AT ALL TIMES, but they do not need to be at your side while you participate.
  
  
• If you are under 12 years old, we must consider your case on an individual basis. Please email cmstwinmask@gmail.com to discuss with our staff.
